Use this form to:
1. Tell us about a new staff member for your TPSID,
2. Request a data entry account for a staff member, and/or
3. Tell us about any other changes to your staff (e.g., staff member left or changed roles).
This information will be used to update the National Coordinating Center's contact list for TPSID personnel and to make sure each staff person has access to the TPSID section of the website.
An account creating access to the TPSID section of the website will be created for every new staff person, and they will get an email with their log in credentials.
If you indicate in the form that the new staff person needs access to the Data Network the new staff person will receive an email with information on required data entry training to be completed prior to being granted full access to the Data Network.
If you only want to tell us about staff persons who have left or changed roles, skip to the last question on the form.
