PARENTS: Thank you again for your interest in my piano studio.  Please answer all the questions in this survey.  This is purely an informational questionnaire; it is not an application or registration for the Studio.  Enrollment into the Studio is only guaranteed by the Studio's acceptance of a signed enrollment form and payment of the first installment of tuition after the Teacher, Parent/Guardian, and Student have held a personal interview.  The information requested in this form is for the Teacher to understand the background of the Student and to understand the Parent's interest in enrolling her/his Student in the Studio.
