If you are under 18 years of age, a parent or guardian must check this box to indicate that they grant permission to the participant to take part in the JUNIOR Reef Keeper program. Any child 13 years or under must be accompanied by a guardian at all times during participation.

As parent/guardian of the above mentioned participant, I understand that myself and my child may be photographed during an event or training activities. These images may be added to Lady Musgrave Experience's portfolio. I give permission for my own image as well as the participant's image to be used in social media and other promotional material.
