The Galway Sexual Health Forum is a new initiative for 2021. It's purposes are to facilitate the provision of an excellent and integrated education, support and treatment continuum for all aspects of Sexual Health through the life course in Galway city and county, and to contribute to Sexual Health strategy and policy.

We are asking organisations to complete this form to audit what is currently being provided and ask what gaps are seen in the provision of Sexual Health services and support in Galway city and county.

This information is being gathered by Sexual Health West and will be held by us and used to create a map of services and gaps.
