We are excited to resume in-person worship at Conshohocken United Methodist Church on September 6th, 2020! For legal reasons, we ask that each person who attends read the below waiver and complete the form. Paper forms will also be available Sunday mornings at church.

 The Church Leadership Team and the Church Board of Trustees have undertaken steps to ensure the safety of all who attend worship services or otherwise utilize Church facilities.  However, precautions CANNOT ELIMINATE THE RISK OF YOU OR YOUR FAMILY MEMBERS CONTRACTING COVID-19.  

Every individual who chooses to attend worship services or otherwise utilize Church facilities is responsible for determining, based on their own health and other factors, whether doing so outweighs the risk. YOU ENTER AND UTILIZE CHURCH FACILITIES AT YOUR OWN RISK.

While present in Church facilities, you must follow the following precautions:

1. If you are sick, stay home.  
2. You must wear a face mask at all times while inside church facilities.
3. You must maintain six-feet distance between yourself and all individuals who are not part of your household.  

These rules have been adopted for the safety of everybody and shall remain in effect until further notice.  IF YOU DO NOT COMPLY WITH THESE RULES AND THE INSTRUCTIONS OF CHURCH STAFF/USHERS, YOU WILL BE ASKED TO LEAVE.

If you determine that you and/or your family cannot attend worship services safely or that you cannot comply with these rules, please contact Rev.
