This form is to be used by an Off Campus Employer hiring a work study eligible student.

COMPLETE AND SUBMIT THIS FORM TO THE WORK STUDY OFFICE BEFORE THE STUDENT BEGINS WORKING. This Work Study Hiring Form should be used in conjunction with an approved Work Study Job Description. Summer work study starts July 1st, 2023. The Work Study Job period for the academic year is October 2nd to June 7th, 2024 unless otherwise specified.

Student should have an email that confirms eligibility to participate for the specific school year. Student can also confirm eligibility and amount of their award by accessing their financial aid information on their MyUW. Note that if there is no work-study value under summer quarter, this means they do not have summer work-study. Do not assume that student has work-study for the academic school year (fall-spring), if you have only verified for summer - vice versa.
