This is the official application form for the 6th cohort of the successful entrepreneurship accelerator in Holly Springs.  Acceptance is based on the quality and completeness of your responses to the questions below, attendance to one information session, and an interview.

Be aware that, once you have applied & been selected, cohort members are expected to do the following:

* Attend 100% of the 3-hour training session that are held on ten consecutive Thursday evenings.
* Complete all assigned homework.
* Participate in a Mentor Matching session to identify your mentor.
* Engage with your assigned Mentor for six months.
* Attend a graduation ceremony at the end of the six-month Mentoring period.
