Thank you for applying to Rise Up's Summer Arts Academy! This form is the first step in getting your child registered for the camp!

Camp dates are: 7:30 AM- 5:30PM Monday - Friday July 26- August 6, 2021.

Free breakfast and lunch will be included for ALL campers thanks to a collaboration with MJUSD.

In order to register your child/ children you will need to fill out this registration form. Once received, our system will email you a link to a pay portal for your specific tuition amount based on our sliding scale and applicable sibling discounts. Your child's spot in the camp will be reserved upon receipt of payment.

Please have some documents ready for the following registration form. You will need: Your child's information (including date of birth, legal name, preferred name, age, dietary restrictions, and medical conditions), Your information (including legal name, preferred name, address, Adjusted Gross Income information from IRS, valid email address, and phone number).
