Agency Volunteer Need Request Form

Local Agencies and organizations that have a need for volunteers are to use this form to submit there request where it will then be transferred to the Michigan Volunteer Registry.

Please be aware that you will be responsible for checking volunteer temperatures prior to each shift and for asking volunteers questions to check for COVID-19 symptoms as to assist with slowing the spread. Any volunteer that is found to not meet these requirements will be thanked for there assistance but are asked to go home.
