Before filling out this application it is required that you log onto A4H's registration system and register for ALL of the programs you wish your child (ren) to participate in. Go to the www.A4HSports.com --> registration - complete instructions to register. Once completed bypass the payment section (your information will save). We will not evaluate applications who have not completed this step.
