Please complete form to register.  Cost is $40 per student.  Payment is due at the completion of the form to reserve your place in class.  Class size is limited so sign up today! Open to ages 8-Adults. This is a beginner/intermediate dance class.  Class takes place at the CMT Rehearsal Space inside the Sierra Vista Mall, 1050 Shaw Ave, Clovis CA. Space is located inside between the Gold Coast Gallery and the Barbershop, closer to the Kohl's end of the mall. Arrive 15 minutes early to check in. Dress in comfortable clothes and jazz or tennis shoes. No open toed shoes, boots, etc. Bring a water and be ready to dance!

Authorization to Consent to Medical Treatment:  I (We), the undersigned, do hereby authorize representatives of Children's Musical Theaterworks of Fresno (such representatives to be employees, directors, Auxiliary members or identified volunteers) to serve as agents for the undersigned to consent to an X-ray exam, anesthetic, medical or surgical diagnosis or treatment and hospital care which is deemed advisable by and is to be rendered under the general or specific supervision of any physician or surgeon licensed under the provisions of the Medicine Practice Act on the medical staff of any hospital licensed by the State of California whether such diagnosis or treatment is rendered at the office of said physician or  at said hospital or some other site. It is understood that this authorization is given in advance of any specific diagnosis, treatment or hospital care being required, but is given to provide authority and power on the part of the aforesaid agent to give specific consent to any and all such diagnosis, treatment or hospital care which the aforementioned physician in the exercise of his best judgment may deem available. I (We) also understand and agree that CMT of Fresno will not be held responsible for injuries which occur to self/child while attending or participating in any CMT of Fresno function. This authorization shall remain valid for the duration of the participant's current registration with CMT of Fresno. *

Waiver of Liability: As the parent/guardian of the above-named child and/or on behalf of myself and/or my child, agents, heirs, and successors, I voluntarily agree to: 1) assume all risks of injury, illness, or death to myself or my child arising out of or resulting from participation in and/or attendance at the above-stated program or activity, such risks to include but are not limited to, injury, illness, or death due to being exposed to or infected by contagious diseases, including COVID-19; 2) waive and release all claims, causes of actions, actions, liabilities, and costs against CMT /Terry Clark Dance and its staff and board and members thereof, officers, employees, agents, and volunteers, and hold harmless CMT and Terry Clark Dance from any claims, causes of actions, actions, liabilities, and costs that may arise out of or result from my child’s participation in or attendance at such program or activity; and 3) assume all obligations for any medical, financial, and other costs and/or liabilities that be sustained or incurred by my child, myself, or my agents, heirs, and/or successors. CMT and Terry Clark Dance assumes no responsibility and shall not be liable for any injury, illness, death, liabilities, damages, or costs that my child, myself, my agents, heirs, and/or successors may sustain or incur arising out of or resulting from the aforementioned program or activity. *

RULES AND AGREEMENTS: 

1) Closed Class: Parents, friends and or family may not stay to watch class.
Children ages 14+ may exit on their own accord only if and when their parent, guardian or driver has arrived.

2. Refund Policy: Payment is due at the time of registration to secure placement. No refunds will be granted under any circumstances due to the limited nature of this event.

3. General Policies
• Respect others. No foul or inappropriate language is allowed.
• Smoking any substance and alcohol consumption is prohibited.
• Cell Phone Usage is NOT allowed during class.
•Only water is allowed in a closable container.
• Clean up after yourself. 
• Appropriate attire is always required. Participants wearing overly revealing or inappropriate clothing will be asked
to change. Dance shoes or tennis shoes are required. Wear clothes you can move in. Jeans are discouraged.
• We is not responsible for your personal items. Leave them at home or bring them at your own risk. Should personal items be lost or stolen we will not be responsible for their replacement.
• Participants must respect and follow all  Staff instructions.
• Participants who do not drive themselves must be dropped off and picked up on time. Staff will NOT stay past a scheduled release time.
• Participants may not touch or operate any of the technical equipment in the rehearsal space or theatre without
permission from the staff.

1) Advertising and Promotional Release:  I hereby consent to the reproduction and/or use of photographs, DVDs, and film and audio recordings of myself (or my child/charge) for advertising and promotional purposes only by Children's Musical Theaterworks of Fresno or its affiliates AND Terry Clark Dance. I have read and agree to all  Policies.
