Farm to Fork Producer Survey
The Utah Farm to Fork Task Force is attempting to improve local procurement in Utah's Child Nutrition Programs by compiling a database of farmers who are interested in selling their products to schools, child care centers, and summer food programs. This database will be publicly available on the Farm to Fork Task Force website. Please complete this short survey if you would like to be listed in the database. If your farm has sites in more than one county, please fill out a separate survey for each location.
