In consideration of being permitted to participate in the Virtual Live-stream session on Zoom with Evan Seah (hereinafter called "The Activity"), I do hereby release, waive, discharge and covenant not to sue Evan Seah (hereinafter called "The Host") from liability of any and all claims including the negligence of The Host resulting in personal injury, accidents or illnesses (including death) and property damage/loss resulting from, but not limited to participating in The Activity. I hereby assert that my participation is voluntary and that I knowingly assume all such risks. *
In compliance to the Personal Data Protection Act (PDPA), I hereby give my acknowledgement and consent to Evan Seah and his administration team the following: (1) Collection and use of personal information for registry of participants and to disseminate information on the meeting details / upcoming events / virtual sessions (2) Photo, sound and video recording during the session to ensure safety of participants. *
