WORKBOOK: Student Workbook must be picked up at 440 Lenox Road, #1J (call 718-270-2983) to arrange pickup (bring a copy of your receipt)
PRETEST: A pre-course assessment (test) should be completed BEFORE coming to class. Print a copy of your results. NOTE: As per AHA, you will not be allowed to take the course without the pre-course. The link for the precourse will be emailed to you after registration and payment.
NOTE: Registration is a 2 step process. After filling out this form, you will need to make payment for the course in order to secure your seat. Failure to make payment in a timely manner may result in the class being closed and you will need to select a new course date. Payment information will be given once this form is submitted.
