There are two parts to the registration, this form, and paperwork that you will receive later. You can complete the paperwork fully online, or fully in person (we recommend online).

This form will take about 15 minutes to complete and must be with the student and at least one parent/guardian present.

Answer the questions using the EXACT formatting when requested. Please use normal spelling and capitalization. If you are completing this at home, answer all questions to the best of your ability (we can clarify and correct any answers after). If you are completing this in person and need assistance, ask for help from one of our volunteers.
