This form is to be filled out for those who are not golfing, but would like to join us for dinner, drinks, auction and dancing on Friday, October 4th at DeLaveaga Golf Course.  Cost for a dinner ticket is $20, with checks payable to the Gino Panelli Foundation and sent to 427 Rigg St. Santa Cruz 95060. Please fill out for each person in your party, and if you're bringing more than 4, simply fill out a second form with the rest of your party.  Thank you, and we will see you on the 4th!
