By Signing this electronic waiver i accept all the club tearms and rules.
On behalf of my child, I accept and assume any and all risks associated with his/her attendance and participation in any and all UNITED ELITE SOCCER programs and activities. I understand that my child should not participate if he/she is not healthy. I understand that my child must abide all rules and the instructions given by the staff of the program. I understand that my presence is required at all sessions my son/daughter is in attendance so, I can care for him/her if injured. Permission is hereby granted for photographs and/or videos to be taken of my child during sessions for publicity and marketing materials use by UNITED ELITE SOCCER. Knowing these facts, and in consideration of you accepting my child's participation in the UNITED ELITE SOCCER program; I, or anyone acting on my child's behalf, agree that UNITED ELITE SOCCER and/or UNITED ELITE SOCCER's staff are not responsible for accidents, injuries, and/or medical or dental expenses arising from my child's participation in this program. In accordance with this agreement, I promise not to sue, and I release UNITED ELITE SOCCER and anyone working on their behalf from all claims of liability or expenses of any kind relating to my child's participation in any and all activities. Lastly, I agree to the three months season agreement and its auto-payment condition. If for any reason I decide to remove my child/children from this program, I must give one month notice and pay for that last month of the program through auto-payment in order to end the agreement before three months. En nombre de mi hijo/a, acepto y asumo cualquier y todos los riesgos asociados con su asistencia y participación en todas y cada una de las actividades y programas de UNITED ELLITE SOCCER. Entiendo que mi hijo/a no debe participar si no está saludable. Entiendo que mi hijo/a debe cumplir con todas las reglas y las instrucciones dadas por el personal del programa. Entiendo que mi presencia es requerida en todas las sesiones en que mi hijo/a este presente y así poder cuidar de él/ella si sufre una lesion. Otorgo permiso para que fotografías y/o videos que se tomen de mi hijo/a durante las sesiones y que estas sean usadas como publicidad y/o material de mercadeo por UNITED ELITE SOCCER. Conociendo estos hechos, y en consideración que UNITED ELITE SOCCER acepta la participación de mi hijo/a en el programa de fútbol; Yo (o cualquier persona que actúe en nombre de mi hijo/a), concuerdo que UNITED ELITE SOCCER y/o el personal de UNITED ELITE SOCCER no son responsables por accidentes, lesiones y/o gastos médicos o dentales que surjan de la participación de mi hijo/a en este programa. En consideracion a este acuerdo; prometo no demandar, y libero a UNITED ELITE SOCCER, a sus empleados, y representantes de todas las reclamaciones de responsabilidad o gastos de cualquier tipo relacionados con la participación de mi hijo/a en cualquiera y todas las actividades. Por último, estoy de acuerdo con el acuerdo de la temporada de tres meses y su condición de auto-pago. Si por alguna razón yo decido retirar a mi hijo/a de este programa, debo dar un (1) mes de aviso y pagar ese último mes del programa a traves de auto-pago para poder terminar en acuerdo antes de los tres meses.
