CymaSpace Access Services Intake Form (do not use)

Thank you for choosing CymaSpace for your access needs! The CymaSpace Access Program provides communication services for art and culture nonprofits, educational institutions, etc. that recognize the need to create accessible and inclusive environments for the Northwest Deaf and Hard of Hearing (DHH) community.

Upon submittal of this form, an Access Program representative will be in contact with you. Please send all inquiries to: access@cymaspace.org.

Please note the following:

1. Please make requests 2 weeks in advance to give us time to recruit providers. Contact us if you need to expedite service and we will work with you. (access@cymaspace.org) We cannot guarantee services for rush requests.

3. Once a request is made it is considered billable by the Access Program on the agreed amount. You must make a cancellation request within 48 business hours before the event. The scheduled service will be billable if recruited interpreters are confirmed.

4. Clients must be timely in providing preparatory materials for Sign Language Interpreters, including copies of any speeches/presentations, lyrics, scripts, cue cards and any media that will be deemed helpful in preparation. Materials must be provided 48 hours in advance of the event or earlier. Send materials to access@cymaspace.org.

5. CymaSpace is not responsible to promote our event to the DHH community. If you are inviting the Deaf community, please mention in your media that you have a "Deaf and Hard of Hearing Accessible Event" so that the DHH community can be aware of your event.
