Informed Consent
INTRODUCTION
You are invited to participate in an undergraduate interior design capstone research study to investigate Improving Healing Spaces: User-Centered Design for Behavioral Health and
Sensory Wellness. Please take whatever time you need to discuss the study with your family and friends, or anyone else you wish to. The decision to join, or not to join, is up to you.
WHAT IS INVOLVED IN THE STUDY?
If you decide to participate this is a basic outline of what will happen over the course of your
participation in this questionnaire, we hope to ascertain information about the places and spaces where therapeutic services are provided within the unit. We anticipate that responses
will help guide the creative agenda based on your suggestions and ideas. We think this will take you 15 – 20 minutes to complete.
RISKS
This study involves no known risks, however, there may be unknown minimal risks that we cannot predict. You may stop taking this questionnaire and close out of the survey at any time.
BENEFITS TO TAKING PART IN THE STUDY?
There are no gifts in kind, monetary benefits or perks to taking this study.
CONFIDENTIALITY
Answers for this questionnaire will be stored on a password protected, multi-factor authenticated drive folder. This survey is anonymous, and the data will be aggregated and represented as data sets. Quotes if any, may be used but will not contain identifying information.
USE OF INFORMATION
Your information will not be used or distributed for future research studies.
YOUR RIGHTS AS A RESEARCH PARTICIPANT
Participation in this study is voluntary. You have the right not to participate at all or to leave the study at any time. Deciding not to participate or choosing to leave the study will not result in any penalty, and it will not harm your relationship with RIT and the Interior Design Program.
