The Sad Moms Club: Personal Story Submission 
Hey friends, this is the place to submit your personal experience of a perinatal mood and anxiety disorder, infertility, pregnancy loss, or infant loss. I ask for your information, so I can contact you in the future, but we can keep it anonymous if you would like.

A few things:
1) If you are chosen this time, I will keep these entries and may reach out in the future depending on how this first episode goes. You can always change your mind if you are not ready to share.
2) For the sake of professionalism, please no names if you had a bad experience with a provider. You're welcome to rave about your provider (doctor, therapist, prescriber, PT, etc.) if you had a good experience though!
3) Try to frame your story in such a way that would have been helpful if you had heard it when you were at the beginning of your journey.
