NOTE THIS IS A 2 STEP PROCESS.  Below is Step 1 where we will collect basic demographics.  When you hit "submit" you will be taken to a link to CORE's registration page, which is Step 2 in the appointment and registration process.  Some basic questions may be repetitive, but MUST be completed in order for your appointment to go smoothly.  Thank you in advance for your patience.

This testing day is operated by CORE New Orleans and the New Orleans Musicians' Clinic. We want to make sure you and your family members have special and safe access to FREE COVID testing so you can stay well and keep the beat alive.  Please spread the word to your community, neighbors and loved ones who may want testing.

***All participants must adhere to social distancing measures and masks must be worn at all times except when the shallow nasal swab is administered.***

Bring the CONFIRMATION NUMBER you receive at the VERY END OF STEP 2 WITH YOU ON TUESDAY FOR IDENTIFICATION.  It should also be emailed to you.
