僑生、外籍生入境臺灣調查表 Overseas Students Re-Entering Taiwan Investigation
為了預防Covid 19 在冬季期間再次因旅遊而傳播，臺灣教育部要求學校調查僑生及外籍學生的入境臺灣的狀態。 若是在2022年12月至2023年3月期間，有入境臺灣的計劃，請填寫下面入境訊息。

In order to prevent  Covid 19 transmission through traveling in winter, Ministry of Education in Taiwan requires the school to investigate overseas students who plan to exit and re-entry Taiwan from the end of December 2022 to the end of March 2023. Anyone who is applicable to the above situation, please fill out the following form.

自主防疫場所 The Place for Self-initiated Prevention *
本校宿舍未符合疾管署規範自主防疫場所條件（1人1室），故無法做為自主防疫場所 。住宿生入境後須於校外實施自主防疫0+7的規範，且於第7天快篩陰性，才可以返回宿舍。

The room-arrangement in our school dormitory do not conform with the principle of self-initiated prevention from CECC. ( 1 person per room with a private bathroom). Therefore,  the 7-day self-initiated prevention CANNOT be taken in dormitories. After arriving Taiwan, NTUT school boarders are required to follow the regulations of self-initiated prevention OFF-CAMPUS.  Boarders CAN only enter the dorm by showing the proof of rapid test negative on the 7th day of self-initiated prevention.
