Parents must remain with and work with their children. The Church will be open for prayer Eucharistic Adoration as well!  This element is optional and may be attended as needed.

               community service projects, etc)

Children are invited to celebrate the sacraments in grade 2-First Reconciliation and Grade 3-Confirmation and First Eucharist. Preparation for the sacraments requires completion of grade 1 Family Formation, Registration in the parish, valid Baptism, and regular Mass attendance.  Proximate preparation begins in the winter of grade 2 with First Reconciliation and then moves into Confirmation and First Communion preparation which bridges over into the fall of Grade 3.

It is expected that families will participate in formation even after sacraments are celebrated.  Your child will receive the Gifts of the Spirit and be fully initiated into the faith at Confirmation.  He or she will be able to receive Jesus in the Eucharist for the rest of life.  In order to sustain a relationship with God, children need to part of a family of faith and a community. These sacraments prepare us to be disciples of Jesus and are just the beginning of a life long journey.  Formation, prayer, service and participation in the Eucharist are necessary to fulfill that mission. And, there is so much more to explore about our faith!

Teens in Grades 6 to 12 have a new opportunity to live out their faith, learn more and contribute to the community.  They will meet at the same time as the younger children but will meet in small groups with a mentor to tackle tough topics, pray, have fun and plan larger youth events. They can also serve as assistants to the younger grades at their sessions. Please register these youth on this form.

The tuition for Family Formation remains the same as it was for PSR. This fee covers the cost of the books and supplies we will use for our gatherings.  $50 per child  $100 for two or more children.  Waivers are offered to families for whom the fee is a hardship.

Please fill out the required registration below. Be sure to scroll all the way down to sign the Photo and Video, and Child Safety permissions.

Permission for Photo and Video Use         Video and still photography may be taken during parish catechetical and youth ministry programs.  This  authorization constitutes parental permission for the children in your family to participate in the videotaping and photography and use of the images in parish, diocesan or secular media.  (newspapers, websites, video and television outlets) *
Child Safety Permission     As you may know, all adults of our diocese who interact with minor children in our parishes and schools are required to attend a Protecting God’s Children Training. Also included in developing a safe environment for our children is the training of students enrolled in our elementary, middle and high schools, as well as the students enrolled in the Parish School of Religion and Home Study programs. St. Agnes Family Formation will be using the Diocesan Required Virtus "Touching Safety" program supplemented by the Family Connections supplements within the Be My Disciples Series and materials from Netsmartz. Families will cover this material during their weekly sessions. We will also offer the "Touching Safety" material a session during one of our Brunch gatherings to complete the Diocesan required materials. *
