Parent Request for Transportation Reimbursement - Form 1000

PARENT INSTRUCTIONS:

(Note: this is a reproduction of an official state form with some added notes for clarity.)

If your child is transported to and from a non-public school at your expense, and is eligible for reimbursement, you may, through your school of attendance, request reimbursement for transportation to the boundary of the district residence. (In other words, the school district in which you reside would transport your child to school, but only to a school in your school district of residence...OLG is NOT in your school district of residence. Your school district of residence may reimburse you for a portion of your transportation costs.)

Minnesota State Law specifies that students who attend non-public schools are due transportation services or reimbursement to the district boundary.

Please submit ONE form for EACH student you are requesting reimbursement for.
