This year's session will be on June 23rd-28th @ Camp Rock Creek. 
This form does not guarantee that you'll be on staff or that you will be given the role that you specified in this form, but we will do as best as we can to plug you in where we think your talents are best fit!  Thank you for taking interest in this session! All staff will need to have a recent background check - Have you had a background check in the last 5 years? If not, you will need to have one for you to be on staff. Please reach out to your eldership and ask if they will take care of this for you.  *
We will be opening the camp up to staff at Saturday, June 23rd @ 6 PM for camp setup. This will be a time for us to setup, hangout with each other to build some relationships, pray for our students, and play some games. We will stay the night at the camp and you will have the morning to go do what you need to do. All staff will need to report to CRC no later than 1 PM on Sunday.

By submitting this form, the participant (or parent/guardian) acknowledges and accepts the risks of physical injury associated with participation in the activity described above, except for gross negligence on the part of the sponsor. The participant (or parent/guardian) accepts personal financial responsibility for any bodily or personal injury sustained during the activity. Further, the participant (or parent/guardian) promises to hold harmless the sponsoring organization and its representatives for any injury related to the activity. I relieve the Southwest Church of Christ from any liabilities due to sickness, accidents, and/or injuries  while attending the camp or while coming to or from the camp grounds. If I cannot be reached by phone, I give my consent to the director or nurse to authorize physicians to administer medical help.
I understand and accept that I release Southwest church of Christ from any accident or incident that occurs during Spark Session. *
