Dear Prospective Agent:

Thank you for your interest in becoming a certified agent with the Women’s National Basketball Players Association. To receive a link to the online application, please complete and submit the form below and send two separate payments to the WNBPA at the address below:

Payment #1: a non-refundable application fee in the amount of two-hundred fifty dollars ($250.00); and
Payment #2: the annual dues in the amount of seven-hundred fifty dollars ($750.00).

Payments must be submitted as either a check or money order, and made payable to the “Women’s National Basketball Players Association." Upon the WNBPA's receipt of your payment and completion of this form, you will be provided with a link to the online application for Certification as an Agent.

Each question must be answered in order for us to fully evaluate your credentials. Failure to provide all required documents will delay the process of your application.

Please note that all Applicants will be required to provide both of the aforementioned payments before the application can be submitted. Please be sure to include the name of the applicant in the memo section of the check or money order.
