Instructions:
We need your help in collecting the emails of your local hospital chief executive officer and/or chief medical officer! With their emails, we will be able to encourage them to either return to universal masking or continue universal masking.
Please fill out one form per person. E.g. Complete this document each time for each leader at each health system.
Tips on finding their contact information:
1. Academic hospitals linked to a university will likely list these leader's emails at the university directory.
2. Premium linkedin users are able to get access to these leader's personal/professional emails through linkedin.
3. Internet searches with their names in quotations, "ABC ABC" and keywords such as hospital name and the word email.
4. Especially physicians, if they have publications, can be searched via google scholar with their primary contact listing their email.
5. Recommendation provided by users - try using
Rocketreach.co, provides limited free access for finding emails.
6. Try searching truepeoplesearch.com - scroll down and check for an institutional email
If you are finding difficulty locating an email - consider looking for contacts of other hospitals outside of your regional network.
