This form is due by August 9, 2024 to receive an AOTA Approved CE Certificate.

If the form is completed after that date or if you complete additional sessions after filling out this form, you will still be issued a CE certificate without the AOTA Approved logo. Majority of states accept non-AOTA Approved CE Certificates for all or a portion of CE requirements.

To receive credit for a course, make sure to fill out the post course quiz on each course.
