This is the formal application for a scholarship for an Endorphin unlimited membership (normally $119/mo). Scholarships/discount will be valid for three months (or until funds are exhausted) and can be renewed if needed/possible. Big thanks to everyone who has contributed directly to the Go Fund Me or to the Auction to make this happen for those who have been financially impacted by COVID. We will notify applicants within one week of receiving the application. Scholarships will be determined based on demonstrated need, history of membership and availability of funds.
