This form is not for students currently enrolled in our district, it is only for new students OR those who attended our district in the past and are transferring back. By completing this form, the parent/guardian certifies they are requesting to register the student named below in the Strafford R-VI School District and that they are legally authorized to make educational decisions for the named student. When registering more than one student, the parent/guardian MUST submit a separate request for each student being enrolled. An email address is REQUIRED to complete the registration forms and to have in the student's file.
