Why are we asking you to complete this form?

We are asking you to complete this form to help us gain an understanding of the many different people we support, and ensure that we address you in the manner that you wish to be addressed.

*We want to make sure that you feel welcomed, understood, and safe.
*We want to understand the people we support.
*We want to ensure our policies and procedures are inclusive of those people we support.
*We want to see where we might have groups of people who we are not supporting and can work towards
   making our service more inclusive and engaging for them.

This way we can work towards meeting everybody’s needs and improving the service.

We need your name so we can record the information in the correct place. This will ensure we can communicate appropriately with you and in a manner that you want us to. We will never share your personal information (name, address, etc) with anyone (professional, parent, guardian) without your permission.

This form is voluntary.  This means you can decide if you want to fill it out or not.  You may just want to fill out certain parts.  If you would like to discuss the form please contact your Young Carer Worker.
