Welcome to a brand new school year, and a special welcome to our Class of 2024! My name is Mrs. Prather and I am your Transition Specialist. I work with all of the students on campus that have an IEP and would much rather be meeting with you in person and getting to see your fabulous face, but here we are in the midst of COVID. The purpose of this survey is to get to know you so that I can help you navigate high school and what TRANSITIONING after high school looks like for you, i.e. College, Trade School or choosing to go right into the workforce. I will then share this information with your Case Manager prior to your Annual IEP Meeting, so they can get to know you better regarding YOUR personal hopes and dreams after high school.
