Internship Expectations (please read & acknowledge)
I UNDERSTAND THAT:

- Both my site supervisor, and I, will be required to submit monthly internship surveys.

- I am required to enroll in (or have previously passed) COLA 402 for academic credit, by federal law.

- If I would like to request accommodations, then I must inform the COLA team and my site supervisor.
