2024-2025 Softball Tryouts 
 Please fill-out this form completely and submit it upon bringing your child to softball tryouts. In addition to this completed form, you will need to bring a paper copy of an updated 2024-2025 physical to the tryout date. Our tryouts will be held on May 30th from 5-7:00 P.M. at the Lake Forest Middle School softball field.  Your child will need all necessary softball equipment (helmet, bat, glove) and a drink.
