Liability Insurance covering this event is required to host an event at the CFC Complex. Insurance shall cover itself and all of its participants, agents, employees, or designees in the event. The limits of such liability insurance shall be not less than Two Million Dollars ($2,000,000), combined single limit for each person/occurrence for bodily injury and/or property damage. Such insurance shall remain in full force during the term of this agreement. Capital Fútbol Club shall be named as additional insured on such policy. User must provide the CFC a certificate of such insurance 14 days prior to the event date.

The undersigned is aware that there are certain inherent risks involved in participating on CFC complex including but not limited to the risk of theft or of damage to property, and the risk of personal injury from participating in athletic activities. In consideration of my being granted permission to participate in these activities and to use the facilities of Capital Fútbol Club, and/or other activities and services provided by the Capital Fútbol Club, its agents and employees, including food service, I, on behalf of myself, my executors, administrators, heirs, next of kin, and successors, hereby covenant to hold harmless and indemnify the City and all of its officers, departments, agencies, agents and employees from any and all claims, losses, damages, injuries, fines, penalties, and costs (including court costs and attorney's fees), charges liabilities, or exposures, however caused, resulting from or arising out of or in any way connected to my or my family’s participation in the above described activity. I have read and understand this HOLD HARMLESS AGREEMENT and by my signature agree to its terms.

GENERAL GUIDELINES & POLICIES

1. Rental fees must be paid in advance. All reservations are considered tentative until we have received payment and a signed copy of this agreement. Tentative reservations are cancelled within five working days of initial scheduling if payment has not been received.
2. Renting groups are responsible for picking up garbage in the areas utilized. The rented facility in the same or better condition than it was prior to rental.
3. Renting groups are responsible for any damage caused to the facility and its contents. Groups are invoiced for all labor and material costs necessary to return the facility to its previous condition. This applies to cleaning as well as physical damage.
4. Games will need to be cancelled if there is standing water on the field.
5. The tractor, Gator, and other equipment will not be used.
6. No admission fee may be charged without prior approval from us. We must also approve the sales of food or merchandise prior to rental.
7. We reserve the right to accept or reject any application.
8. All vehicles must be parked in authorized areas surrounding the facility.
9. If park rules are violated continued use will not be permitted and additional dates will be for¬feited without refund of fees paid.
10. No alcoholic beverages are permitted unless served by Capital FC staff.
11. No Smoking or Tobacco use
