Even though Google requires you to login, I don't know your email address. This is only to prevent people from repeatedly filling in the form to skew the results. There will be two sections of this Google form: the "math level" section, and the "feedback" section.
The original intent of this Google form was to collect your math level so that I could better cater for the audience. I made this form while my channel was small, not thinking that I would even make such advanced videos that I am mostly known for now (c.f. my earlier videos). So, currently the "math level" section doesn't really help me too much in deciding the video topic, though you are still welcome to fill in those. Mostly I just go with whatever I feel interested in at the time.
However, the "feedback" section is still useful. It will be about what you think is good and bad about the videos, and possibly give me some video requests. The start of the complex analysis video series shows that I do read those requests!
