2020 Summer Camp Registration
Welcome to the Fun Days by the Bay Registration portal.
Kindly ensure you read and respond to ALL the questions below.

*Please note that we will be offering sports clinics and a band program this year for an extra fee.  

STEPS FOR REGISTRATION:
1. Register camper below - Every child attending camp must be registered individually.
2. Print out, read, and sign Summer Camp Participation & Release Form.
3. Gather the following documents and submit these to the ILS Camp Office - forms may submitted in person or via email to ilscamp@ilsroyals.com
    a. Legible copy of passport or birth certificate
    b. Original FL Depart. of Health: Florida Certification of Immunization form (Form DH680)
    c. Original FL Depart. of Health: State of Florida School Entry Health Exam (Form DH340)
    d. Camper picture (face shot)
    e. Any report card from the 2019-2020 school year
4. Prepare form of payment in cash, credit card, cashier's check or money order made out to Immaculata-La Salle High School. NO personal checks accepted. Payments may be made by phone with credit card.
5. Visit ILS Camp Office with payment and ALL required forms.
   
Camp Office hours:
Monday - Friday from 8:00-12:00am --- 1:00-3:00pm
