Consent for Juniors
Consent for under 16s, please read carefully:

I understand the activities the Club will offer and agree to my son/daughter/the paddlers names in this form taking part in these activities. I confirm to the best of my knowledge that my son/daughter/the paddler does not suffer from any medical condition other than those listed below. I consent to my child receiving medical treatment, which in the opinion of a qualified medical practitioner, may be necessary.

I agree to be at the pick up point / drop of point at the agreed time.

I understand that the Club or Organisers accept no responsibility for loss, damage or injury caused by or during attendance on any of the clubs organised activities except where such loss, damage or injury can be shown to result directly from negligence of the Club or the Organisers. (Training sessions will be supervised by coaches and other competent paddlers).

Please confirm that you understand that your child must bring a change of clothing appropriate for paddling activities which will get wet, especially a light, windproof cagoule and old trainers!   If your child wears glasses please provide a retainer/strap as if they fall off they will most likely sink.   *
Your child should arrive no earlier than 10 minutes before their slot and should be picked up on time. Can your child walk home without being collected? *
Please confirm that you understand we will collect and store your details until such time that you request to be removed. *

Personal Information which you supply to ESKC may be used in a number of ways, both by us and the Scottish Canoe Association, (the sport's governing body to which we are affiliated).

For example;

* To administer and manage your booking, your information will be held securely by us

* To review, develop and improve the services and activities offered by us and the Scottish Canoe Association.

* To provide you with details of forthcoming activities, events, and competitions (as well as those of selected third parties (including our partner organisations)

* To research, develop and manage new and existing programmes and projects and to promote our sport

* To monitor the Scottish Canoe Association’s progress towards demonstrating equal opportunities in canoeing in Scotland through the Equality Standard for Sport.

* For statistical analysis.

* We may pass your personal information (which may include information about your health) to the insurer in the event of our club making an insurance claim. Information provided by you may be put on a register of claims and shared with other insurers to prevent fraudulent claims.

Your email address will be used by the club will use this to contact you regarding Club activities, events, competitions and membership.

Your child’s space will be secured upon receipt of the £95 payment and a receipt will be issued by our payment system.  If you wish to donate an additional £5 to our Young Carers project, please select the £100 payment option. This will help to fund our Young Carers joining us free of charge. Thank you very much.
