The below registration form has been developed for ST. MARY & ST. BISHOY CHURCH congregation members as an opportunity to participate in the Divine Liturgies every Sunday. It is intended to fairly distribute participation in accordance to the guidelines that are in place by the state.

Family members wanting to attend Liturgy together should complete ONE Registration Form and indicate the number of family members that will be in attendance with them. If family members are attending on a different Sunday, they will be required to complete separate Registration Forms.

ST. MARY & ST. BISHOY CHURCH cannot guarantee all that enter the church are free of the VIRUS. You and/or your family are choosing to come knowing and assuming all risks

يجب على أفراد الأسرة الراغبين في حضور القداس معًا إكمال نموذج تسجيل واحد فقط والإشارة إلى عدد أفراد الأسرة الذين سيحضرون معهم. إذا كان أفراد العائلة يحضرون فى أيام أحاد مختلفة ، فسيُطلب منهم إكمال نموذج تسجيل منفصل

كنيسة السيدة العذراء مريم والأنبا بيشوى لا تضمن كل ما يحضر إلى الكنيسة خالاً من الفيروس . أنت أو أفراد الأسرة مختاراً أن تأتوا عالمين ومتحملين اذا كان هناك أية مخاطر

1- I myself and/or my family have not recently been in close contact with anyone who tested positive within one month
2- I myself and/or my family have no new symptoms of High Fever, Dry Cough, Difficulty Breathing.
3- I myself and/or my family have not traveled by plane or driven to designated areas with HIGH COVID-19 cases within the past 14 days

أقر أنا أو عائلتى لا يوجد عندنا حرارة عالية او كحة او صعوبة بالتنفس
أقر أنا وعائلتى لم نسافر بأية وسيلة للأماكن التى بها نسبة كبيرة من حالات فيروس كورونا - كوفيد 19 - فى خلال 14 يوماً
