"Uptick B" Reservation Form

PayPal deposit must be paid to fill reservation. Use the PayPal button on her webpage if you didn't already to get here! Once both this form is completed and his deposit are paid, your name will be added to an online queue. You can use this queue to keep updated as to when your balance will be due. Balances will only be invoiced when copies are in hand ready to ship. No earlier payments accepted.

Queue identities will be created using the first 5 digits of your transaction id and your initials (e.g. 12345mk). Be sure to refresh your browser to make sure you're looking at the most recently updated due dates. The balance will be invoiced to your PayPal'ed email as they are ready to ship.

The casting and shipping may be take some time, anticipating doing no more than 50 per week, more often it's only 25 per week. So your patience is appreciated!
