填妥申請表格後，請於面見日或前，交回下列文件予校務處  After completing the application form, please return the following documents to the  Office on or before the interview day   
  1. 出生證明書正、副本   Birth certificate-Original and photocopy
  2. 四個回郵信封 (請貼上足夠郵資、申請人姓名及地址)Submitted four stamped self-addressed envelopes, specifying the name and address of the applicant
  3. 報名費四十元正 Application fee HKD $40 現金或支票 cash or cheque    支票抬頭：聖德蘭幼稚園  Cheque payable to “St. Teresa’s Kindergarten”
本人茲聲明以上所填資料乃本人自願提供及明瞭以上資料乃進行此項申請之一般程序，並清楚明白如 貴園獲取錄的學生，資料將會保存於園內成為學生資料的部份，若未能取錄的學生，此表格會銷毀。 *I certify that I have read the school application procedures, and that the information provided on this application is complete and accurate. I also understand the personal information of successful applicants will become part of the school’s student file,in which the personal information will thereafter be handled by the school staff only, and the personal information of unsuccessful applicants will be destroyed.
