WLHS Former Student Transcript Request Form

If you graduated within the past 5 years, please complete this form and pay $5.00 for each transcript requested. Payments must be submitted online at this link.

If you graduated more than 5 years ago please contact student_transcripts@hcpss.org for assistance.

Release of Student Records

The law requires that schools receive written permission signed by the parent/guardian before transcripts, including mid year reports and other student records can be released to third parties.

By completing this form, you are verifying that this form has been completed with your parent/guardian(s) approval if you are under 18 years old.

Please note that when you complete and submit the WLHS Former Student Transcript Request Form you are giving HCPSS permission to send the transcript through email or US Mail.

It may take up to 20 school days to complete your request. 

Please note we are unable to email official transcripts to private email addresses, only to colleges/universities, scholarships. Only unofficial transcripts are permitted to be emailed to students.
