Spartan Track and Field Recovering Athlete Report
If you would like to request a lighter or alternative workout, or a complete day off of practice due to a diagnosed or suspected injury, illness, etc., you need to fill out this form ASAP, and before the start of practice if you feel like you need additional attention.  Please do not SKIP practice.  You should first speak to a coach about your issue.  In most situations we will prepare an alternate workout, rehab therapy, or other tasks you can do to help the team.

A copy of this form must be filled out EVERY DAY you're requesting consideration for your injury/soreness/concern.  The only exception to this rule is if a long term plan has been agreed upon with your coaching staff.
