Thank you for volunteering your time with Stars! If you are driving while volunteering, please read below and electronically sign this form, agreeing that you meet the requirements to drive and will follow Stars' Transportation Policy.

- Drivers must have and provide proof of a valid driver’s license of the State they reside in.
- Drivers must have and provide proof of current car insurance
- Drivers must have current registration.
- Drivers must follow all rules of the road (speed limit, no cell phone use or distracted driving, everyone using their seatbelt, etc).

In the past three (3) years, drivers should have no more than:
 -- Two (2) At-Fault Accidents
 -- Three (3) Minor Moving Violations, (personal and/or business)
 -- Four (4) Non-Moving Violations, such as:
 -- Failure to Appear (FTA),
 -- License not in possession, or
 -- No Proof of Insurance/Registration
 -- Four (4) or more in combination of the above a, b, and c.

In the past four (4) years, drivers should have zero (0) Major Moving Violations such as:
 -- Driving while Suspended/Revoked and/or Invalid License
 -- Exhibition of Speed - Speed Contest
 -- Reckless Driving
 -- Driving Under the Influence (DUI)
 -- Vehicular Manslaughter
 -- Leaving the Scene of an Accident (Hit and Run)
 -- Speeding in excess of 100 mph, etc.

When driving Stars student(s):
 -- Drivers should have at least two years driving experience and be at least 18 years old.
 -- Any child who is eight years or younger, or under the 4 feet 9 inches tall must sit in the back seat with a
    federally-approved child passenger restraint system.
 -- It is illegal to leave a child six years or younger unattended in a motor vehicle unless they are left with a child
    12 years of age or older.
 -- It is advisable to never be alone in the car with a child of the opposite gender.

Understand that in the course of your work as a volunteer, you may be exposed to medical, financial, or personal
information pertaining to participants and/or families. You understand and agree that this information is to be
kept strictly confidential and will not be shared with anyone except Stars Staff who need to know the
information in order to provide services. You further understand that divulging the information may result in your
immediate dismissal from your volunteer role with Stars.
