This is a short form to record the outcome of all rapid flow tests.  Completing this form automatically updates our live spreadsheet tracking test results.  If you test POSITIVE please make sure you ring school straight away for advice and a follow up conversation on close contacts.  If the result is not during school hours please email the Headteacher on cpatterson@friesland.derbyshire.sch.uk
If your test is VOID, please try again with another test.  If it is also VOID please contact school to ask for advice.  If your test is negative please complete the form and no other action is required.
