The purpose of this questionnaire is to ensure we have the necessary information to guide you in the right direction. There are so many variables that come into play when we’re looking at health, wellness and performance and this allows us to get a full picture of where you’re at currently.
This questionnaire will also help you become intentional about how you want to benefit from coaching and assist in you in structuring your thoughts and clarify your goals.
As you work through it, here are some things to keep in mind:
- Take your time. Ponder the questions. We find that it’s best to fill this out during a slow morning with a cup of coffee where you can be present and uninterrupted. Sit down, appreciate the moment, be open and let those next 30+ minutes support you in becoming excited for what is ahead of you.
- Some questions are more general and some are a bit more specific. We need a blend of both to paint the full picture. The more detail you can provide in your responses the better.
- There may be some uncomfortable things that come up for you. I encourage you to into it and be as open, honest and transparent as you can be. Any answer to any question is YOUR answer. It reflects where you are in life and where you’d like to be. No right or wrong.
