This form only needs to be filled out by one group member. You must fill out this form AND get your purchase approved by your RAD mentor BEFORE the due dates. Purchases are due at 8pm the Friday of weeks 4,5,6,7, and 8. Purchases will be processed and sent within a few days after the Friday deadline.
