Non-liability, Consent, and ReleaseThe Mid East Performance Association (MEPA), and its respective contractors, board of directors, staff, and representatives, will not be held responsible for any injuries sustained by any performing members, staff or support personnel of the named group (listed below) during or in conjunction with any performances or attendance at any performances of the MEPA competition season.
Furthermore, the below named group and its sponsoring organization hereby agree to indemnify, defend, and hold harmless the MEPA and its respective employees, board of directors, staff, and representatives, from any and all claims which might be brought by any person associated in any way with the named group, including performing members, in any way arising out of or during any performance at any MEPA event during the competition season.
This specifically includes, but is not limited to, any injuries sustained by performing members, staff or support personnel caused by equipment utilized by the group. I do hereby certify that I have read these provisions and am duly authorized to execute and approve this document on behalf of the named group. Furthermore, I hereby acknowledge that all of the performing members and their parents, staff, and support personnel will be notified of the provisions of this Notification of Non-liability, Release, and Consent Form, and will be provided with a copy of this form upon request.
Media Release I consent that MEPA or photographers designated and assigned by MEPA may use or permit use of in any and all media formats and may copyright for sales, advertising, publicity, or electronic use images and performer names during the competition season. I certify that I have informed performing members and parents or legal guardians of performers 18 years of age and under of this release and have gained their permission. Any performing member requesting their images and/or name not be used has been listed below using their legal name.
I certify that any promotional material sent to MEPA by the person listed as “Group Representative” below may be used in all promotional capacities regardless of any exemption listed below.
Contact Release
I consent that MEPA may release my email address only to trusted and confirmed circuit sponsors. Additionally, said sponsors would contact me only, not the performing members or their parents, guardians, adults of my group, via email mailing list, and I will have the option to 'Unsubscribe' from any sponsor's marketing materials should I feel that it is not necessary for my group named below. I am not obligated in any way to use the sponsor's services or products if I so choose, and that they are available for me as a convenience and resource.
