Welcome to the FCBC Children's Program Registration for 2021-2022! This form will guide you through the entire process to register your children from birth through sixth grade.

This form will collect Parent/Guardian Information first, Emergency Contact Information, and Medical / Insurance Information before it asks you about your children. Instead of filling out multiple forms, you can now fill out just one single form and register up to five children in various programs.  The form is valid for August 1, 2021 - July 31, 2022.

歡迎你在此註冊報名，以便你的孩子參加2021-2022年度FCBC兒童活勳，這表格會逐步指引你如何為你從出生嬰兒至6年級小孩完成整個註冊過程。

本表格會先收集家長/監護人資料、緊急聯絡電話，以及醫療/保險資料，然後問及你的孩子。你無需填寫多份表格，現在可以用一份表格為一至五名孩童報名，參加多項活動。本表格的有效期為2021年8月1日至2022年7月31日。
