Canyon View Church MOPS Fall 2021 Registration Form
Thank you for your interest in joining MOPS! After a crazy year and a half, we are excited to meet again in person on Wednesdays at 10am. Please fill out the form below to be included in our gatherings including childcare. The cost to attend MOPS Fall 2021 is $90 per mother and an additional $10 per child who will need childcare. This will include your subscription to MOPS International. Once you have completed the registration form please go to canyonview.org/contribute-now, select "Donate", add an optional note with "MOPS and your name" and follow the steps to complete your payment. Please list the name, age, date of birth and any allergies of each child that will need childcare during our meetings. When paying your contribution for MOPS please add an additional $10 per child.
Thanks again for your interest in joining our MOPS group! We will reach out shortly with more information on our Fall Semester beginning Wednesday, September 15th. Don't forget to complete your registration by submitting your donation of $90 per mother and $10 per child to canyonview.org/contribute-now. Canyon View MOPS wants to include any mother who wishes to join our group regardless of her financial situation. Because we believe that all requests for assistance should be honored on a true need, we offer a limited number of scholarships.
